You may re-order by:

  • Ticking items needed on request slip (attached to your prescription) and bringing to the surgery. Please post in the post box at the side of the building marked Ellenbrook Medical Centre.
  • Post – send request slip (attached to your prescription) with SAE
  • Email to Please note this email is for ordering prescription requests only.
  • Online via the link at the top of this page - in order to use this service you must have previously obtained the necessary security access codes. Please call at reception for details. Please bring a photo I.D.

Children under 14 will not be allowed to collect prescriptions.

Electronic Prescribing

The Practice offers the Electronic Prescribing Service (EPS). This means you can nominate a pharmacy that we can send your prescription to electronically, removing the need for you to collect the paper prescription at the surgery. To find out more about the service or to sign up please call in to the pharmacy of your choice to discuss EPS.  From October 2020 if you have not nominated a pharmacy to have your prescription sent to electronically then you will be required to collect a token from the surgery.  This is an unsigned prescription that can be taken to any pharmacy to collect your prescription.

Medication Reviews

Patients on repeat medication may be asked to see a doctor, nurse practitioner or practice nurse for a medication review. If this applies to you a note will appear on your prescription to notify you of this.
